A media conference discussing the progress of the investigation

At the request of the Queensland Government, the Australian Transport Safety Bureau has dispatched a team of Transport Safety Investigators to conduct an investigation into the Tilt Train Accident near Bundaberg which occurred in the early hours of 16 November 2004.
The ATSB's report into the Epping rail accident reinforces the limitations of reliance on automatic train stops and 'deadman's handle' devices in the event of driver incapacitation and highlights the importance of improving these defences as well as medical standards and processes for train crew.

The Australian Transport Safety Bureau has found that heavy fog and the inappropriate speed of a truck in the conditions were the main contributors to a collision with a freight train at the Lismore Skipton Road level crossing at Lismore, Victoria on 25 May 2006.
The ATSB has found that a collision between a freight train and motor car occurred because the flashing lights, bells and boom gates failed to operate as the train approached the level crossing.
The ATSB has found that a fatal collision between The Overland passenger train and tip truck towing a tri-axle trailer occurred because the truck driver probably did not see the train and entered the level crossing after braking too late to stop at the 'Stop' sign.
The ATSB has found that a collision between a train and low loader truck occurred because the truck driver was probably distracted and did not see the train approaching.
The ATSB has found it is likely that some passive level crossings in the Northern Territory, Queensland, Western Australia and South Australia that are controlled by 'Stop' signs and are used by high combined gross mass road vehicles may have deficient sighting distances.
The ATSB has found that insufficient train braking and inadequate warning distance contributed to a collision between a track mounted excavator and a freight train at Inverleigh, Victoria on 25 September 2006.
