The ATSB is investigating an incident where crew were injured during maintenance activities on the engine room of the oil tanker Wisdom Venture, near Sydney, New South Wales on the 6 May 2025.
At 1400 local time, while awaiting berthing, the crew were conducting maintenance/service work on the engine room, when the main steam isolation valve released high temperature condensate water resulting in burn injuries to three crew members.
As part of its investigation, the ATSB will collect evidence from relevant parties and analyse relevant records and documents.
A final report will be released at the conclusion of the investigation. Should a critical safety issue be identified during the course of the investigation, the ATSB will immediately notify relevant parties, so that appropriate safety action can be taken.
