The ATSB is investigating an engine failure involving a Robinson R44, VH-OOE, near Tindal Airport, Northern Territory, on 11 June 2025.
During cruise, the pilot experienced severe shaking and vibrating through the airframe. The pilot conducted a precautionary landing into a field resulting in minor damage to the airframe and no injuries. A post-flight inspection found the engine had a mechanical discontinuity, resulting in major damage to the engine.
The evidence collection phase of the investigation will involve:
- interviews with involved parties
- disassembly and examination of the engine
- examination of maintenance records
- retrieval and review of recorded data
- collection of other relevant information.
A final report will be released at the conclusion of the investigation. Should a critical safety issue be identified during the course of the investigation, the ATSB will immediately notify relevant parties, so that appropriate safety action can be taken.
