The ATSB is investigating two flight control events involving Embraer E190-100, registered VH-UZD, near Launceston Airport, Tasmania, on 15 April 2025, and Canberra, Australian Capital Territory, on 18 April 2025.

On 15 April 2025, the flight crew received a slat failure caution and multiple associated alerts. After an unsuccessful system reset attempt, the crew diverted the aircraft to Melbourne, Victoria, and conducted a flapless, zero slat landing. The engineering inspection found that a component in the left outboard slat system had been incorrectly assembled. The system was subsequently rectified and returned to service.

On 18 April 2025, the flaps failed to deploy, and the flight crew conducted a missed approach. The flight crew diverted the aircraft to Melbourne and after landing received a brake overheat alert. Aircraft maintenance engineers carried out fault isolation checks and reset the flap system. Additional precautionary inspections were carried out prior to the aircraft being ferried to Adelaide where one of the left flap actuators was replaced.

A final report will be released at the conclusion of the investigation. Should a critical safety issue be identified during the course of the investigation, the ATSB will immediately notify relevant parties, so that appropriate safety action can be taken.
