The Australian Transport Safety Bureau has commenced a transport safety investigation into a collision between 2 trains that occurred on 17 August 2022, at Oonoomurra, Queensland.

At about 0645, Qube train 9277 entered the loop line at Oonoomurra to allow Aurizon train 9M48 to pass on the main line. However, the Qube train was not clear of the main line, and the locomotive of Aurizon train 9M48 collided with 9277. The locomotive of 9M48 derailed and the driver received minor injuries. 

To date, the investigation has included:
- examination of the accident location
- interviewing the train crew
- analysis of the recorded information

In the course of the investigation, the ATSB has identified risk controls that potentially contributed to the occurrence. Examination of these factors represent an increase in the scope of this investigation, and it has been upgraded from Short to Defined as a result (the ATSB's different levels of investigation are detailed here).

The continuing investigation will include further analysis of:
· operational processes and procedures
· crew training
· Emergency procedures 
· related occurrences.

A final report will be published at the conclusion of the investigation. Should any critical safety issues be identified at any stage during the course of the investigation, the ATSB will immediately notify relevant parties so appropriate safety action can be taken.
