A NSW Trains passenger service (C012) collided with an abandoned car on the rail line near an active level crossing at Kembla Grange at approximately 0410 on 20 October 2021.
The first car of the train derailed and tipped onto its side as a result of the collision. There were injuries to three passengers and the train crew. The train driver and guard were transported to hospital. There was no driver in the car at the time of the collision.
The Office of Transport Safety Investigations (OTSI) deployed an investigator on behalf of the ATSB under the Transport Safety Investigation Act 2003 to gather on-site evidence. Further evidence will be sought from NSW Trains, Sydney Trains and other interested parties to determine the factors contributing to the incident and other safety factors concerning the road rail interface, crash protection and emergency management.
A final report will be released at the conclusion of the investigation. Should a critical safety issue be identified during the course of the investigation, the ATSB will immediately notify relevant parties, so that appropriate safety action can be taken.
