An investigation has been launched into a collision with a buffer stop by passenger train 150E at Richmond Station, Sydney, NSW at approximately 0953 on 22 January 2018.

As part of the investigation, investigators will attend the accident site, examine the train and the track infrastructure, interview the crew, and gather additional information including the train's data recorder.

A final report will be released at the end of the investigation. Should a critical safety issue be identified during the course of the investigation, relevant parties will be immediately notified so that appropriate safety action can be taken.

This investigation is being led by the New South Wales Office of Transport Safety Investigations (OTSI). Under the provisions of the Transport Safety Investigation Act 2003, the ATSB will provide OTSI with technical and analytical support.
