The ATSB is investigating an unstable approach involving a Jetstar Airways Airbus A321, VH-OYF, at Sydney Airport, New South Wales, on 26 June 2025. The aircraft was operating as Jetstar Airways flight JQ38, scheduled from Denpasar, Indonesia, to Sydney.

During an approach to land at Sydney Airport, the approach became unstable, and the crew commenced the published missed approach procedure. During the initial climb, the crew received an alert as a result of dual control input. The crew subsequently continued the published missed approach procedure, and the aircraft landed without further incident.

The evidence collection phase of the investigation will involve interviewing the involved parties, examination of operational documentation, retrieving recorded data, and the collection of other relevant information.

A final report will be released at the conclusion of the investigation. Should a critical safety issue be identified during the course of the investigation, the ATSB will immediately notify relevant parties, so that appropriate safety action can be taken.
