The ATSB is continuing a transport safety investigation into the derailment of PN train 82P7 and subsequent collision between Aurizon train 9F02 and containers from the derailed train, at Marmor, Queensland, on 29 January 2023.
At about 1902 local time, while travelling on the main line, a Pacific National freight train experienced an axle fracture and derailment. Subsequently, Aurizon train 9F02, travelling in the opposite direction on the adjacent line, collided with containers from the derailed train. Nil injuries were reported.
To date, the ATSB investigation has:
• interviewed involved parties,
• analysed event recorder data,
• examined rolling stock and maintenance records, and
• examination of the fractured axle.
In the course of the investigation, the ATSB has identified potential limitations in risk controls and organisational factors relevant to the occurrence. Examination of these factors represent an increase in the scope of this investigation, and it has been upgraded from Short to Defined as a result (the ATSB's different levels of investigation are detailed here). The continuing investigation will include further analysis of:
• related occurrences,
• operational documentation, and
• processes and procedures for inspection and maintenance of rolling stock.
The ATSB released a preliminary report from this investigation on 6 April 2023. A final report, which will detail contributing factors, will be published at the conclusion of the investigation.
However, if at any time during the investigation the ATSB identifies a critical safety issue, the ATSB will notify the relevant stakeholders so that they may take appropriate and timely safety action.
