The ATSB is investigating an occurrence where a lack of coordination resulted in multiple aircraft operating in close proximity near the Tenterfield and Wallangarra fire ground on 31 October 2023.
It was reported that due to the lack of coordination, the crew of a number of aircraft reported multiple aircraft operating in close proximity.
The ATSB has commenced the examination and analysis of the initial evidence collected.
To date the ATSB has investigation has included:
- interviewing a number of flight crew and coordination personnel
- a review of multiple operating procedures
- analysis of ADS-B data
- review of multiple written incident reports
- review of multiple logbooks of key coordination personnel.
In the course of the investigation, the ATSB has identified potential limitations in risk controls relevant to the occurrence. Examination of these factors represent a significant increase in the scope of this investigation, and it has been upgraded from Short to Defined as a result (the ATSB's different levels of investigation are detailed here).
The continuing investigation will include further analysis of:
- procedures
- inter-agency operations
- recorded data
- communications.
A final report will be released at the conclusion of the investigation. Should a critical safety issue be identified during the course of the investigation, the ATSB will immediately notify relevant parties, so that appropriate safety action can be taken.
