The ATSB is investigating an in-flight break-up involving a Robinson R66 helicopter, registered VH-KFT, east of Hawks Nest, New South Wales, on 26 October 2023.

The aircraft departed Cessnock toward Newcastle then tracked north following the coast. Shortly after passing the Yacaaba Headland the aircraft suffered an in-flight break-up and collided with the ocean near Hawks Nest NSW.  The aircraft was destroyed, and the pilot fatally injured.

To date, the ATSB investigation has included:

  • analysis of recovered recorded data
  • analysis of environmental conditions at the time of the accident
  • analysis of aircraft contributing factors

In the course of the investigation, the ATSB has determined that there to be a reasonable likelihood of limitations in risk controls and organisational factors relevant to the occurrence. Examination of these factors represent an increase in the scope of this investigation, and it has been upgraded from Defined to Systemic as a result (the ATSB's different levels of investigation are detailed here).

The draft report internal review process has been completed. The draft report has been distributed to directly involved parties (DIPs) to check factual accuracy and ensure natural justice. Any submissions from those parties will be reviewed and, where considered appropriate, the text of the draft report will be amended accordingly.

Following the external review process, the report is approved by management before being sent to the ATSB Commission for final approval. Once approved, the final report is prepared for publication and dissemination and released to DIPs prior to its public release. 

The final report will be released at the conclusion of the investigation. Should a critical safety issue be identified during the course of the investigation, the ATSB will immediately notify relevant parties, so that appropriate safety action can be taken.
