The Australian Transport Safety Bureau (ATSB) has commenced a transport safety investigation into a collision with terrain involving a Hughes Helicopters 269C, registered VH-OBK, in Mentone, Victoria, on 30 November 2022.
A student pilot departed Moorabbin Airport, Victoria to conduct a solo training flight in VH-OBK. About 1 nautical mile from returning to the airport, the helicopter collided with the roof of a suburban house. The pilot was seriously injured, and the helicopter was destroyed.
As part of the investigation, the ATSB will interview the pilot and involved parties, examine any available recorded data, review relevant procedural and maintenance documents and records and collect other necessary evidence.
A report will be published at the conclusion of the investigation. However, should any critical safety issues be identified at any stage during the course of the investigation, the ATSB will immediately notify relevant parties so appropriate safety action can be taken.
