...non-restricted documents only held by the Australian Transport Safety Bureau relating to the accident involving Robinson R44 II helicopter VH-ZGY on 12 July 2019.

Specifically, I request the following non-restricted documents:

1. All internal ATSB correspondence (including emails, file notes and memoranda) relating to the investigation.

2. All correspondence (including emails, letters and file notes) between the ATSB and the Civil Aviation Safety Authority (CASA) relating to the accident.

3. All correspondence (including emails, letters and file notes) between the ATSB and the Western Australia Police Force relating to the accident.

4. All photographs, diagrams, site sketches, factual records and inspection records relating to the accident or wreckage that are not publicly available.

5. All flight tracking and flight data relating to the accident involving VH-ZGY on 12 July 2019, including any Spidertracks/Spidertrack data, flight path maps, tracking logs, GPS coordinates, telemetry data, and any analysis or reconstruction of the aircraft’s flight path.

6. Any documents referring to or discussing:

• a rope, strap or sling being over or attached to a helicopter/skid;
• dynamic rollover;
• the cause or mechanism of the accident; or
• the identification, location or recovery of the aircraft.

7. Any document schedules, indexes, registers or lists identifying documents held by the ATSB in relation to this investigation.

8. Any documents referring to or discussing any insurance claim arising from the accident involving VH-ZGY, including communications with insurers or loss adjusters and any documents provided to or received from them.

For the purposes of this request, I do not seek access to third-party personal information or documents that would require third-party consultation where that can reasonably be avoided.

The following documents relating to a crash incident VH-LAI at Kurnell in January /February 2023. The incident occurred in Class C Airspace under the approached of active runways at YSSY.

1. The provision of the pilot or operator incident report provided to the ATSB detailing information leading up and including the accident sequence of the crash .

2. Airservices transponder logs of the time of the reported incident to verify the aircraft was operating and in contact with the Sydney terminal ATC.

1. Documents held by the Australian Transport Safety Bureau relating to complaints, reports, referrals, enquiries, correspondence or other matters submitted by, concerning, or referring to me, between 1 October 2025 and 14 June 2026.

This request includes, but is not limited to, documents relating to matters associated with CASA complaint references COMP0004717 and COMP0005117, and any related referrals, enquiries, reports or communications.

I seek access to:

• Internal emails, file notes, assessments, briefings, attachments, records of decision and working documents.
• Communications with CASA, Airservices Australia, NSW Police Force, NSW Police Aviation Command/Air Wing, Department of Defence, Australian Defence Force, Surveillance Australia, Leidos Australia, Commonwealth departments, State agencies, ministerial offices, Members of Parliament or electorate offices concerning those matters.
• Documents recording referrals, consultations, discussions, assessments, reviews, decisions, actions or recommendations arising from those matters.
• Documents identifying information obtained from, provided to, exchanged with, discussed with or verified through external agencies concerning those matters.
• Documents relating to aircraft activity, aviation safety concerns, surveillance aircraft, Dash-8 aircraft, Surveillance Australia, Leidos Australia, Wattle Grove, Bankstown or any matters associated with correspondence submitted by or concerning me.
• Documents containing references to "[Applicant name]", "Wattle Grove", "[XX] Wattle Grove Drive", "Bankstown", "Surveillance Australia", "Leidos", "CASA", "Airservices Australia", "NSW Police", "Air Wing", "Defence" or "ADF" in connection with those matters.

2. Documents relating to my inquiry concerning a reported Near Mid-Air Collision (NMAC) / AIRPROX event over Wattle Grove, NSW on 23 November 2025 at approximately 18:38 AEDT.

This request includes documents relating to my correspondence with the ATSB and ATSB officer between 30 March 2026 and 2 May 2026, including:

• Internal emails, file notes, assessments, attachments, briefings, records of decision and working documents.
• Records of internal database searches conducted in relation to the reported event.
• Records of searches conducted using FlightRadar24, ADS-B Exchange or any other flight-tracking, radar or aviation data sources.
• Documents recording the methodology, findings or outcome of those searches.
• Documents relating to material supplied by me concerning the reported event.
• Communications with CASA, Airservices Australia, NSW Police, NSW Police Aviation Command/Air Wing, Surveillance Australia, Leidos Australia, Department of Defence, Australian Defence Force or any other agency concerning the reported event.
• Documents recording whether any notification, referral, consultation or enquiry was made with any responsible person under the Transport Safety Investigation Act or Regulations.
• Documents recording why the ATSB concluded that no further investigation would be undertaken.

I am seeking access to existing documents and records only and exclude personal information and documents already in my possession.
