ME-2012-009

Summary
On 25 July 2012 a close quarters incident occurred between two ferries, a fishing vessel and an Australian Navy vessel off Bradley Head, Sydney harbour NSW.
The Office of Transport Safety Investigation (OTSI) New South Wales was responsible for investigating this occurrence. The OTSI requested assistance from the Australian Transport Safety Bureau (ATSB) in the display and presentation of vessel positional data. To assist the OSTI and protect any information supplied, the ATSB initiated an External Investigation under the Transport Safety Investigation Act 2003.
The OTSI provided the ATSB with Automatic Identification System (AIS) data and radar vessel positional data. The ATSB subsequently used the Avenca Marine Accident Data Analysis Suite (MADAS) to display and present the position of the respective vessels and provided that information to the OTSI investigators.
The OSTI has finalised its investigation of this occurrence, and a report is available from the OTSI website, or from the OTSI directly.
