| Safety issue description |
Training of new personnel on board Matthew Flinders III did not adequately cover the use of the different steering modes. Additionally, drills were not conducted in line with regulatory requirements, which meant critical crew had not participated in any steering drills. Training and drills focused on hydraulic failures and did not include the use of the emergency toggles nor practice local control in the engine room. These elements likely contributed to the lack of systems knowledge among the ship’s crew, resulting in an incorrect response to the failure. |

| Action description |
Bass Strait Freight (BSF) has implemented changes in ship-specific familiarisations for Matthew Flinders III. Every crew member is required to complete a crew systems and anchoring competency assessment under the supervision of an experienced crew member or instructor. This assessment explicitly includes the switchover procedures for the different steering modes. In addition, BSF has appointed an experienced master as check captain and introduced a structured captain competency assessment, that is to be completed by each master under the supervision of the check captain. |
