| Action description |
Sekigahara Seisakusho advised the ATSB that the RTM Weipa incident was the first reported incident of its kind. Due to the large number of such cranes installed over the years, the manufacturer assessed that no modifications were necessary to the crane design. In respect of RTM Weipa’s crane, Sekigahara Seisakusho advised ASP Ship Management that the 4 securing bolts for the traversing motor were to be reinstalled using a high-strength thread locking adhesive, ‘Loctite 262’, and tightened to a torque setting of 16.6–20.6 Nm. On 31 March 2026, Sekigahara Seisakusho confirmed having published a service news bulletin on its website, which summarised the RTM Weipa incident and included a revised visual inspection procedure for verifying the traversing gear integrity at monthly intervals.8F In accordance with the 2026 SOLAS amendments,9F it has also proposed to distribute the service news bulletin to ships (crane users) as part of an updated maintenance standard. In addition, Sekigahara Seisakusho has recommended that a manufacturer-certified technician undertake a shipboard inspection of these cranes at least once every 5 years.
