Safety issue

Investigation number MO-2025-006
Safety issue description

Shipboard defects were not recorded in the designated planned maintenance system but instead, were routinely conveyed through an ineffectively implemented crew handover process. Consequently, defects were inadequately addressed and ASP Ship Management lacked oversight of any corrective measures required.

Issue status justification

ASP Ship Management (ASP) has implemented fleetwide training measures at various intervals to reinforce the importance of accurately recording defects in the planned maintenance system (PMS). ASP has also reiterated that alternate means of communicating defects, including through crew handover notes, cannot replace the process of PMS defect logging. Additionally, the PMS has been updated to reflect major overhaul inspections on the crane, which will be carried out at the ship's dry docking. ASP shore-based management will continue to monitor the effectiveness of these measures through periodic reviews and inspections. Collectively, the actions taken are assessed as having adequately addressed this safety issue.

Action description

On 3 September 2026, ASP Ship Management (ASP) reported having implemented the following fleetwide corrective and preventive actions to ensure accurate recording, handover, follow-up and shore-based oversight of shipboard defects:

1. Post-incident crew refresher training was provided in the correct use of the PMS’s defect reporting capability, the company's defect reporting procedures, and the handover process between shipboard management ranks. The training reiterated that verbal and written communications, and crew handover notes were not to substitute the process of recording defects in the PMS.

2. Crew pre-joining briefings were amended to include guidance on the defect reporting process and individual responsibilities for reporting and following up outstanding defects. Crew were also required to be briefed on the status of defects applicable to the ship they are joining to support the continuity of information handover.

3. Reinforcement of the above guidance was carried out during the company's sea staff seminars in November 2025 and May 2026.

4. The shipboard defect reporting process is now being verified by ASP shore management during ship inspections and internal audits. This shore-based oversight is also being used to address any gaps in crews' understanding of procedures or implementation of ASP's safety management system.

5. A 3-yearly PMS task has been created for a major overhaul of the crane to be carried out at every intermediate docking. The last such overhaul was completed in July 2026, during the ship’s dry docking in China. The inspection covered the crane’s mechanical, electrical and structural components, wire ropes, brakes, gearboxes, limit switches and safety devices. The crane was subsequently inspected by Lloyd’s Register, the ship’s classification society.
