Safety issue

Investigation number AO-2025-073
Safety issue description

The required fuel quantity indicator calibration card was not displayed in 2 of Flylink Aviation College’s DA20 aircraft. In addition, it was regular practice to use the fuel totaliser in place of the fuel quantity gauge to crosscheck expected fuel remaining, removing an independent check of fuel on board.

The installation of a calibration card will allow pilots operating the aircraft to correlate gauge readings with fuel on board. Furthermore, new procedures and checklist items will ensure that flight crew crosscheck fuel assessed through other means against the fuel quantity indicator both preflight and during flight, incorporating an independent check of fuel on board.

Action number AO-2025-073-PSA-01
Organisation Flylink Aviation College
Action description

Flylink Aviation College arranged a fuel quantity indicator system test and installation of a calibration card for its 2 DA20 aircraft that did not carry the card. In addition, it issued a notice to aircrew advising of new procedures being incorporated into company checklists requiring pilots to:

  • check that the fuel gauge placard was on board the aircraft before flight
  • crosscheck dipstick readings against the fuel gauge and placard during preflight inspection
  • communicate and agree on the amount of fuel on board using the method above
  • calculate endurance in-flight with reference to the fuel gauge and placard rather than the fuel totaliser alone.
