Sharp Airlines conducted an internal investigation into the incident and identified that the rate of the pilot’s speech likely contributed to the incident. As a result, it issued a safety advisory to flight crew, reiterating the importance of effective radio communications. Sharp Airlines also reviewed and updated its risk register and re-assessed the risks associated with ineffective communication at non-controlled aerodromes. Further, Sharp Airlines have introduced a procedure to ensure that any discussions around the identification and assessment of hazards is now documented in safety meetings. In response to the draft report, Sharp Airlines conducted an assessment of the placement of antennas on its aircraft and pilot radio selection at non-controlled aerodromes, with detail incorporated into the risk register. |
