Pressurisation events involving Embraer ERJ 190, VH-XFM near Alice Springs Airport, Northern Territory, on 3 and 4 June 2026

Summary

The ATSB is investigating 2 aircraft pressurisation events involving an Embraer E190, registered VH-XFM, being operated by Airnorth from Alice Springs, Northern Territory, to Perth, Western Australia, on 3 and 4 June 2026.

On 3 June, during climb at 18,000 ft, the aircraft depressurised and the crew conducted an emergency descent to 10,000 ft before returning the aircraft to Alice Springs. The engineering inspection revealed a hole in the pack 2 duct. During the subsequent flight on 4 June, the aircraft again depressurised on climb, at 28,000 ft, and the crew conducted a precautionary descent before returning the aircraft to Alice Springs. The engineering inspection revealed damage to the conditioned air check valve.

The evidence collection phase of the investigation will involve examining the history of the damaged components, as well as relevant maintenance information and service bulletins.

A final report will be released at the conclusion of the investigation. Should a critical safety issue be identified during the course of the investigation, the ATSB will immediately notify relevant parties, so that appropriate safety action can be taken.
