The ATSB is investigating two incorrect configuration events involving Embraer 190's. VH-A2T, near Olympic Dam Airport, South Australia, on 4 February 2025 and VH-A2V near The Granites Airport, Northern Territory, on 8 March 2025.

In both events, during approach, the crew selected and landed with an incorrect flap setting.

In the course of the investigation, the ATSB has identified potential limitations in risk controls that potentially contributed to the occurrence. Examination of these factors represent an increase in the scope of this investigation, and it has been upgraded from Short to Defined as a result.

A final report will be released at the conclusion of the investigation. Should a critical safety issue be identified during the course of the investigation, the ATSB will immediately notify relevant parties, so that appropriate safety action can be taken.
