The Australian Transport Safety Bureau is investigating the collision involving the container ship Maersk Shekou and the sail training ship STS Leeuwin II in Fremantle inner harbour, Western Australia.

Maersk Shekou was being conducted to its berth by a Fremantle harbour pilot with four tugs in attendance when it collided with the berthed STS Leeuwin II at about 0617 on 30 August 2024. There was substantial damage to STS Leeuwin II and its two crew members were injured.

To date, the ATSB has:

  • interviewed the vessel’s master and crew, both pilots, tug skippers and VTSOs
  • reviewed:
    - recordings of relevant communications and written communications between various parties
    - bridge recordings
    - Fremantle Pilots and Fremantle Ports procedures
    - Bureau of Meteorology data
    - the vessel’s logs and records.

An interim report, which detailed factual information established during the evidence collection phase, was released on 17 April 2025.

The ATSB has completed the evidence collection and analysis phases of the investigation and is drafting the final report.

The final report will be released at the conclusion of the investigation. Should a critical safety issue be identified during the course of the investigation, the ATSB will immediately notify relevant parties so appropriate and timely safety action can be taken.
