Accredited Representative to the NTSB - Engine failure involving a Vans RV-4, registered N423WH, at Sadler, Texas, United States, on 7 December 2020

Summary

On December 7, 2020, at 1528 US central standard time, a Vans RV-4 airplane, N423WH, was substantially damaged when it was involved in an accident near Sadler, Texas. On approach to land, the pilot decided to initiate a go around. While the engine initially responded, the engine then rolled back to idle power. The aircraft landed, overran the runway and flipped over. The pilot was seriously injured.

The US National Transportation Safety Board (NTSB) investigated this occurrence. As Australia was the State of Manufacture of the fuel injection unit, the NTSB requested appointment of an Accredited Representative from the ATSB.

To facilitate this request, the ATSB initiated an external investigation under the provisions of the Transport Safety Investigation Act 2003. The ATSB activity for this investigation is now complete.
