The ATSB is investigating an Air Traffic Control procedural error involving a SAAB 340B, VH-ZLH, at Perth Airport, Western Australia on 9 May 2018.

The aircraft was issued a clearance to conduct a standard instrument departure (SID) from runway 03. The aircraft was subsequently cleared to taxi and take-off from runway 06.

As part of the investigation the ATSB will be gathering recorded audio, interviewing controllers and flight crew and gathering additional information.

A final report will be published at the conclusion of the investigation. Should a critical safety issue be identified during the course of the investigation, the ATSB will immediately notify relevant parties so appropriate safety action can be taken.
