Safety Issue
The Australian Maritime Safety Authority did not appropriately
assess the risks associated with the leakage of dangerous goods on
board Kota Pahlawan until 19 June 2006, more than one day
after the master had reported it and before the ship started its
transit of the Great Barrier Reef Inner Route.
Safety Recommendation
The Australian Transport Safety Bureau recommends that the
Australian Maritime Safety Authority take action to address this
safety issue.
