Safety Issue

The maintenance for a critical piece of equipment, the main lubricating oil pump, was not planned according to the manufacturers recommendation, despite the fact that a previous failure had occurred.

Safety Recommendation

The Australian Transport Safety Bureau recommends that AJ Ship Management takes action to address this safety issue.
