Aircraft Maintenance Engineers in Australia'.
The Australian Transport Safety Bureau recommends that
maintenance organisations introduce clear error-reporting policies
in order to encourage staff to report incidents related to human
error. Such policies should set out in advance the consequences
which will result should maintenance personnel report that they
have made an error.
