The Australian Transport Safety Bureau (formerly the Bureau of Air Safety Investigation) recommends that Rolls Royce Commercial Aero Engine Limited review Rolls Royce RB211-524D4 engine cold stream nozzle inspection criteria to minimise the possibility of failure during operation.

Response text:

1) A world wide communication was sent to all RB211-524D4 operators, Boeing and Rolls-Royce resident service representatives on 06 January 2000 alerting them to this incident, subsequent findings by the affected operator, the position of the cracks and the proposed method of inspection.

2) A mandatory Non Modification Service Bulletin (NMSB), RB211-78-C955, has been issued detailing the inspection of the affected area, the actions required should cracks be found and the availability of a method of repair.

The need for future inspections will be decided when the results of further analysis and laboratory tests become available and should subsequent inspection be necessary a further issue of the NSMB will be made.
