The ATSB is investigating a track obstruction, due to loss of freight, involving train 6WM2 near Glenrowan, Victoria, on 30 March 2018.

At 1450 EDT, V/Line passenger service 8615 made an emergency stop following the driver's observation of an obstruction on track. The obstruction was identified as a large steel coil. Contact was made with Pacific National freight train 6WM2 that had passed through the area earlier. The freight train stopped and following an inspection of the wagons it was reported a steel coil was missing.

Damage to both tracks (east and west), along with damage to multiple wagons on the Pacific National train was reported due to the loss of the steel coil. No injuries were reported as a result of the emergency brake application of the V/Line train.

As part of the investigation, the ATSB will obtain reports from the operators, interview the crew and gather additional information.

A final report will be released at the conclusion of the investigation. Should a critical safety issue be identified during the course of the investigation, the ATSB will immediately notify relevant parties so appropriate safety action can be taken.
