At about 2222 on 12 February 2017, Aquadiva was transiting the main shipping channel under pilotage departing Newcastle harbour when it moved out of the channel. The laden bulk carrier was manoeuvred back into the channel with the assistance of the attending tugs. The ship then continued its voyage to its next port. Subsequently, the Port Authority of New South Wales reviewed the incident and hydrographic data, which indicated that the ship contacted the seabed while it was outside the channel.

The ATSB has commenced an investigation into this occurrence and is collecting relevant evidence, including interviewing directly involved parties. The evidence will be analysed and a draft investigation report compiled. This draft report will be forwarded to relevant parties for comment prior to the completion and release of the final report.
