At 1612 on 17 December 2015, the leading car of empty Sydney Trains passenger service 109D derailed at No. 534 points after the train reportedly passed signal HY82 at STOP, without authority (SPAD). Service 109D was bound for No. 3 Platform at Hornsby station to commence a run to Central station. There were no reported injuries.

The ATSB has commenced an investigation into this incident, which will be undertaken by officers from the NSW Office of Transport Safety Investigations, under the provisions of the Transport Safety Investigation Act 2003. Investigators have commenced collecting evidence and statements from involved parties, which will be analysed and a draft investigation report compiled. The draft report will be forwarded to relevant parties for comment prior to the completion and release of the final report.

The investigation is continuing.
