Summary

On Thursday 3 December 2015 at about 0545 (WST), loaded ore train M03544 derailed  near Walla, Western Australia, approximately 68km south of Port Hedland on the BHPB WA Iron Ore mainline. On inspection it was found that 21 ore cars had derailed. There were no injuries to the train crew, however around 200 metres of the West track was substantially damaged.

The ATSB has initiated an investigation into the incident, in accordance with the provisions of the Transport Safety Investigation Act 2003.
