Safety issue description

V/Line did not adequately address level crossing sighting issues at B. McCann Road acknowledged by the rail operator in 2009.

The Australian Transport Safety Bureau recommends that V/Line reviews its processes and implements improvement in the follow-up of identified level crossing sighting deficiencies.

Response text:

V/Line has reviewed its processes for the follow-up of level crossing sighting deficiences. The role of a Rail Interface Coordinator has been established that will have responsibility for ongoing liasion with road authorities with respect to level crossing interfaces.
