Safety issue description

SPAD Investigation Form (RS-ADL-283) used by Public Transport Services does not collect data on many of the human factor issues that would facilitate a better understanding of why SPADs are occurring.

Public Transport Services has advised that the following action is proposed with respect to collection of SPAD data. ... that it will review its SPAD Investigation Form to ensure that human factors issues are addressed as part of the initial SPAD Investigation process.
