At about 1135 on 17 November 2011, a crew member fell overboard
from the container ship MSC Siena, off Fremantle, Western
Australia. The man had been rigging a pilot ladder when the
accident occurred. A search for the man was carried out by the
ship, several nearby boats, water police boats, helicopters and
aircraft deployed by Australian authorities. The lifejacket that
the man had been wearing, his boots and lifebuoys deployed from the
ship were found during the extensive search but his body was not
recovered.
On 18 November, two ATSB investigators travelled to Fremantle,
where the ship was berthed, to conduct an investigation. The
investigators interviewed involved parties and collected relevant
information and evidence. The evidence obtained will be analysed
and a draft investigation report compiled. When the draft
investigation report is approved for release, it will be forwarded
to directly involved parties for comment.
