Safety issue description

High service time stage-2 LP turbine blades were susceptible to a reduction in fatigue endurance as a result of vibratory stresses sustained during operation at speeds close to the maximum.

The manufacturer is recommending that operators replace high service time stage-2 LP turbine blades.

ATSB response:

Replacment of high time blades should reduce the likelihood of blade failure.
   
Current issue status: Adequately addressed
Status justification:

Will ultimately reduce the likelihood of LP2 blade failures. In the meantime, the risk of another complex failure is very low.
