Pacific National (PN) operated Melbourne to Perth freight train 4MP5, was being propelled during a shunting movement inside PN's yard and passed yard departure signal 214U displaying a stop indication. The shunting movement continued for a distance and then derailed on a set of catch points before colliding with the scheduled Sydney to Melbourne RailCorp operated eXpress Passenger Train (XPT), numbered 8622, running on the main line.

Actions Taken

As a result of the occurrence, the ATSB consulted with Pacific National and the Australian Rail Track Corporation in relation to safety issues that had became apparent during the initial stages of the investigation and issued interim recommendations. The interim recommendations are contained in 5.1.1 and 5.1.2.

The ATSB also published on 20 May 2005, an Interim Factual Rail Safety Investigation Report in relation to this occurrence.
