Wreckage evidence indicates that helicopter's main rotor struck a tree at about 25 feet above the ground. About 4 feet of a rotor blade separated followed immediately by a severe mast bump which broke the mast. The main rotor assembly departed the helicopter. The helicopter then crashed into a tree before impacting the ground.
