Main-track derailment and collision of freight trains, Alberta, 21 April 2012

The report into the derailment of a freight train on a Canadian Pacific Railway siding south of Innisfail, Alberta, was released on 25 June 2013. Citing a combination of factors including the track that was not level near the turnout and a mismatch in the height of couplers between cars, the investigation found that the wheels on the first derailed car likely climbed over the rail and began a derailment that involved 10 cars and a locomotive.  Continue reading

Bell 206 helicopter terrain collision near Angusville, Manitoba, 5 July 2012

The report into a controlled flight into terrain of a Bell helicopter engaged in fungicide spraying operations was released on 21 June 2013. No malfunction in the aircraft system was found.  The report found that although certified and qualified for the flight, the pilot had a low experience level for the complexity of the task.  Continue reading

Loss of separation at Vancouver, Richmond, British Columbia, 15 April 2011

A Dash 8 took off, adjacent to the intersection of the Echo taxiway at Vancouver International Airport. Moments later, a Boeing 737 was cleared for takeoff from the threshold of Runway 26L. A third aircraft, a Boeing 737 which was on short final for landing on Runway 26L, was directed to carry out a missed approach. It was dark at the time, and all three aircraft were on instrument flight rules flight plans. The two Boeing 737s came within approximately 2,000 feet lateral spacing before instrument flight rules separation was restored.  Released on 20 June 2013, the report found that confusion by the air traffic controller and fatigue may have been factors. Track workers nearly struck by passenger train near Paekakariki, North Island

On Thursday 25 August 2011, an electric metro passenger train was travelling southbound from Waikanae to Wellington when it nearly struck two track workers who were repairing the track in a protected work area. The investigation focused on safe work practices, training and prevention of the use of performance-impairing substances. Continue reading

Boeing 737, Birmingham International Airport on 21 September 2012

The Boeing 737 left the paved surface of the taxiway and came to rest on grass having turned to vacate the runway at approximately 20 kt ground speed. The commander was attempting to vacate the runway expeditiously to avoid causing the following aircraft to go around. Continue reading

Runway overrun, Fan Jet Falcon 20E at Durham Tees Valley Airport, UK on 9 August 2012

The aircraft overran the runway when takeoff was abandoned due to a potential birdstrike. No aircraft faults were found to have contributed to the incident although the surface friction characteristics of the runway stopway adversely affected the deceleration rate achieved during the final stages of the rejected takeoff. The lack of a CVR or FDR severely limited the ability of the investigation to determine the exact sequence of events during the incident. Two Safety Recommendations are made. Continue reading

Sinking of fishing vessel Viking II, 75 nm southeast off Cape May, New Jersey, USA

On 7 October 2012, the 64-foot-long fishing vessel Viking II sank about 75 nautical miles off Cape May, New Jersey. The three crew members, a captain and his two sons were rescued unharmed. The National Transportation Safety Board report of 14 June 2013 found that the probable cause of the sinking was uncontrolled flooding of the engine room.  Continue reading

Engine room fire on board Patrice McAllister at Lake Ontario near Prince Edward Point, Ontario, Canada on 27 March 2012

The vessel Patrice McAllister, with six crew members, experienced an engine room fire. The crew released CO2 from the fire suppression system into the engine room and extinguished the fire, however, the fire later reflashed and burned out of control. The vessel's chief engineer was fatally injured, and the five remaining crew members suffered minor injuries. The report released on 6 June 2013 found that the probable cause of the fire was the ignition of lubricating oil that sprayed from an engine pre-lubrication oil pump that came in contact with the manifold. The actions of the crew and the amount of combustible material on the vessel also contributed.  Continue reading
